Our shared goal is simple: to give patients and families the best possible care experience while keeping patients safe, supported, and healing at home—especially after surgery or hospitalization.

Evolution Home Health Inc. provides coordinated, patient-centered home health care, including skilled nursing visits for medication management, clinical monitoring, long-term IV antibiotic therapy, and comprehensive wound care treatment. We also provide physical therapy and occupational therapy to support post-operative and post-hospital recovery, helping patients regain strength, mobility, and independence after surgery or serious illness.

In addition, our skilled nurses place strong emphasis on patient education and infection prevention. We provide teaching and hands-on support for:

• Central line care and catheter care to reduce infection risk
• Wound care and early signs of infection
• Medication safety and adherence
• Support and education for patients who are newly diagnosed with diabetes, helping reduce the burden on patients and families during recovery

Our team works closely with physicians and case managers to ensure clear communication, early intervention, and smooth transitions of care, with the goal of keeping patients safe at home and preventing avoidable readmissions.
